Title | Using Behavior Therapy Incorporating Exposure and Response Prevention to Treat Obsessive-Compulsive Symptoms Comorbid with a Patient with Schizophrenia: Effectiveness, Limitations, and Pharmacotherapy Coordination |

Summary | There is a high prevalence of obsessive-compulsive (OC) symptoms comorbid with patients with schizophrenia. Schizophrenic patients with OC symptoms have exhibited severe depressive symptoms, often attempted suicide, suffered from severe social dysfunction, have had a low quality of life, and had long hospital stays. As a result, the treatment of OC symptoms in these patients is extremely important. This study reported on a case in which a female patient in her early 30s with schizophrenia and olanzapine-induced OC symptoms was treated with behavior therapy incorporating Exposure and Response Prevention(ERP). Behavior therapy was shown to improve OC symptoms related to harm avoidance. OC symptoms related to not-just-right experiences or incompleteness, which were judged not to improve with only behavior therapy, were shown to improve by behavior therapy and gradually discontinuing treatment with olanzapine. As a result, the patient’s OC symptoms went into remission. The case was discussed, and observations were made regarding precautions that need to be taken when using ERP to treat OC symptoms comorbid with patients with schizophrenia. |
