概要 [Objective] To identify disease-specific metabolites, comprehensive metabolomic analysis of patient serum was performed.
[Methods] Serum samples were obtained from 6 healthy controls, 28 patients with IgA nephropathy (IgAN), 28 with autosomal dominant polycystic kidney disease (ADPKD), and 28 with ANCA-associated vasculitis (AAV). Samples were analyzed using capillary electrophoresis time-of-flight mass spectrometry (CE-TOFMS). Correlations with disease-specific metabolites and clinical parameters (age, sex, renal function, proteinuria, etc.) were assessed using Spearman’s analysis, and their predictive ability was further evaluated by multiple regression.
[Results] A total of 640 peaks were detected, of which 271 were assigned candidate compound names from the HMT metabolite library. Heatmap analysis confirmed distinct metabolic profiles across the four groups. Forty metabolites critical for group separation were identified; among these, four in IgAN, eight in ADPKD, and eight in AAV were classified as disease-specific. Correlation analysis between these disease-specific metabolites and clinical indicators revealed that histidyl-glutamine (His-Gln) was significantly elevated in ADPKD and inversely correlated with kidney volume. This association remained significant in multivariate analysis adjusted for age, sex, and eGFR.
[Conclusion] Serum His-Gln levels were elevated in ADPKD and decreased with increasing kidney volume. His-Gln may serve as a biomarker of disease progression in ADPKD.
